"I am confused where I have to go."This sentence is natural to you? I think we need "I do not know" or something to make it natural. What do you think? Thank you so much.

It's okay by me.

You can always add a preposition, "I am confused about where I have to go," but in casual conversation we often skip it.

I don't know where to go.
I don't know where I'm supposed to go.
I'm confused about where to go.
I'm confused about where I'm supposed to go.

As an aside, you might want to check the meanings.
In my opinion, there's a difference in meaning between confusion and ignorance.
Granted, people often mix them up.
Consider "I don't know where to go" vs. "I don't know which way to go."
The first one expresses ignorance, and the second one could express confusion.
With confusion, there's often conflicting information, or some difficulty in interpreting information.
With ignorance, it may be simply that "nobody told me where to go."

"I'm confused" is a state of mind, just like "I'm surprised!".
What has surprised me? What has confused me? What has put me into this state of mind?

But this is not really what you mean. It's illogical.
You're saying "I'm confused because I'm confused." (I'm confused that I'm confused.)
Why am I confused? What am I confused about?
Surely I'm not confused about the fact that I don't know where to go! I'm very clear on that!
I'm not asking myself, "Why is it that I don't know where to go?"
