Hyphens and commas

Would 88-year-old be hyphenated in the following sentence, and is the comma placement correct?

Because the patient is an 88 year old and has emphysema, he is a poor cadidiate for surgery; however, there are other options available.

Yes, I would hyphenate it:

"The patient is an 88-year-old."

"The patient is 88 years old."
