Hyphens

Grateful if someone would let me know which one is correct (and why):

Her pain is cardiac-related.

Her pain is cardiac related.

There is little agreement on things like this. Either way seems all right to me, being equally intelligible. As an adjectival attribute , I would hyphenate it: a cardiac-related problem.
