Hurl Vulgarities

Dear teachers,

May I have your opinion on the following sentence?

He started to hurl vulgarities and threw punches at her.

1. Is "hurl vulgarities" idiomatic?

2. Should there be a comma after "vulgarities"? (= "...vulgarities, and threw punches...")

3. At the same time, I am wondering whether "threw" should have been "throw" without the comma (suggested in question 2).
