for which I'm sure is not the case.

The Roman letter should be indicating the number "4" instead of the "1"-"5" as you've typed out.

But my question is about how you say it, in the most professional way.

I don't joke around with others' questions here. And Mr. M has verified my first response. I simply don't see IV as a Roman numeral, but rather as a (very common) abbreviation of the legitimate medical term "intravenous". Sorry, I had no intention of taking your question lightly.
