How / What ... like

I would like to know if there is any difference between these two sentences:

“He doesn’t know how things really are.”
“He doesn’t know what things are really like”.

Thanks,

There is a clear difference that pops up in my head: He doesn't know how things really are being. He doesn't know what things are really like concerning their appearance. Your welcome, Orgypus

Hello,

I think first one refers to the apprehension of the state of the things(quality).
How things realy are?Bad, good...
and the second refers to comparative understanding of their natures. (hope, I am right)
Cheers
