Hi.

I am filling out a recommendation form for someone, and in that form, I don't know what to write in response to how well?

The follwing is part of the form;

1. How long have you known the applicant? for three years how well?--------------

2. In what capacity have you known the applicant? his B.Sc. advisor and TA supervisor

I would be grateful if you could list me several possible answers for how well?

For instance if I write I have been his supervisor then what should I write for the answer of in what capacity -----?

Both are stilted in AmE.

Since there is a small space for writing how well? in that form so I can think of

1. Completely in touch

2. Quite well

3. Fully/well aware of his performance

4. completely

5. know perfectly well
