I'm trying to figure out the difference between:
(1) How was she supposed to know that...?
(2) How was she to know that...?

No difference whatsoever. In this case, was to = was supposed to.

Yes, still the same. I have never heard #3, which would probably be phrased "How was she to know? / How was she supposed to know?"
