How to write an explanation of failure of exam or essay reference problem ?

Hi guy, recently I've received that I failed my resit exam and also had the problem on my essay(high percentage of matched content) because my gradma passed away just at the week before I took this exam and submited this essay. I got quite high mark on the rest of subjects and I've told this to my course manager and she showed very understandable for this and asked me to write a formal report to explanation of this issues. I've no idea how to write it so wonder if anybudy can help with it. Thx.

Assume you are talking face to face with someone who does not know anything about the problem.

First, explain what happened. Then explain why it happened.Then say what you want to happen now.

Just write it the way you would say it.
