How to write a permission letter to government for family illness???Please help!!!

Pls help me write a letter, i want to write a permission letter to the hospital( the head) where they post me for clincal experince that i will not be coming because am sick and my grand papa am living with is also sick and his son that is living in abroad want to take him to where he is staying for medical check up and me too because know one to take care of him there.
