How to write a model dialog

Hi,

I want to write a model dialog for a hospital visit. Can you help? Tell me about the style, format and others.

(go into the hospital)

Receptionist: Hi, welcome to XX Hospital. Please seat down and fill out this patient in-take form. When you have finished it, please put the completed form in the box labeled "Intake Forms."

Patient: Sure, thank you.

(completes the in-take form and put in the box)

Receptionist: Mr. Smith, the doctor is ready to see you. Please go into his office.

(he goes into the doctor's office)

Doctor: Please take a seat. OK, I see the form you handed in. Are all the information in the form correct?

Patient: Yes, it is.

It should be Is all the information in the form correct? (As 'information' is an uncountable noun 'Is' should be used.)
