Receptionist: Hi, welcome to *** Hospital. <<< receptionists in such places are not usually so polite, as they tend to be busy and stressed. Please fill out this form and put it in this box.

Patient: Sure, thank you.

(completes the in-take form and puts in the box)

Receptionist: Mr. Smith, the doctor is ready to see you. Please go in.

(he goes into the doctor's office)

Doctor: Please take a seat. She usually just expects you to sit down without an invitation.

OK, I see the form you handed in. Are all the information in the form correct?She wouldn't say any of this. Does she expect you to say 'No, I filled out the form incorrectly'?

She'd just look at the section of the form that describes your problem, and then begin to question you about it, eg 'How long have you had this pain in your chest?'

It should be Is all the information in the form correct? (As 'information' is an uncountable noun 'Is' should be used.)
