How to write a formal letter to my boss asking update for my increase (please correct below)

Dear Mr. XXX,

Hope you are doing well.

I know you are very busy, but it has been three months and I have received no changes on my salary, allowance (housing & transportation) and incentive/bonus which we have discussed during your last visit in (city) office on (date).

I would really appreciate for any update.
