How to verbalize the ref's statement

In a soccer match when the referee announces the substitution of players, how would he say ? For examples :

"Player A out, player B in" ? or
"A(lastname) out, B(lastname) in" ? or
"Player B replaces player A" ? or
"Player A is replaced by player B" ?

I think referees usually pantomime, because a whistle in their mouth.

I'd agree: first the coach makes the referee aware that he wants to make a substitution; then (during a stoppage) the referee signals to the coach and waiting substitute; then player A goes off; then player B comes on.
