How to use PASS and FAIL correctly?

Hi all,
I am actually getting confused when trying to use the word PASS or FAIL. Please tell me which one from the following is the correct usage. If you would explain more details, I am very appreciated: .
1> The device is passed ( or failed)
2> The device is passed (or failed) the test
3> The device passed (or failed)
4> The device passed (or failed) the test
5> The device is pass (or fail)
, I went through an examination: 6> The teacher passed (failed) me
7> The teacher let me pass

A PASS
B FAIL
9> Items > Result
A PASSED
B FAILED

Agreed, but I'd say that 9 is also correct. In both 8 and 9 the heading should be "Item" rather than "Items".
