It's the question form of

I was (supposed) to know by some means the way in which the story would go.

by some means becomes the question word "How?", i.e., "by what means?"
the way in which becomes which way.

How I was (supposed) to know? becomes How was I to know?
(Inversion of subject and verb because it's a question.)
