I have troube with one test.

Note that your first one should be TB, not TV, I believe, but also that the new words are the same as the old words. Did you write that one correctly?

    Your sentences are fine except for 5 & 6.

    For 5, you could be correct with "It is he who does not work enough TO pass his exams," but I don't think that was what was supposed to be emphasized. I would have said "It is his lack of work that causes him (or leads him to) not pass his exams" or maybe "It's because he doesn't do enough work that he fails his exams."

    If I were a good cook, I would have invited them to lunch.

    Indeed, Tanit's #4 is superior to yours - the tense is better.

    I don't think there's much difference in #8, but conditionals have never been my strong suit.

    As for no. 8, students are generally expected to write standard types of conditionals, so I guess Anex' sentence would be the correct choice in a test.
    I posted the one with the mixed conditional because I was wondering if native speakers would perceive any difference between them. My idea was that type 3 (If I had been a good cook...) might imply th
