How to tip in Canada or US dinner?

Hi there,

When I finished my eating in a dinner, I asked the girl who served me to bring the bill. When she brought the bill, she asked to me to pay over the counter.

But the person from the counter was not the girl; so in such a situation, where you guys would leave the tip? What I did was leaving the tip on the table where I ate, and walked to the counter paying the food. Do you think this is good? Anyway, what's the normal way to do this?
