How to say this?

Hi,

Here is a condition:

I am at my friends' house. He's not home and after a while he gets back. I want to ask him where he went. So how do I ask it ( how to ask this in spoken english). Is it correct to ask: "Where have you been ?"

I just got home from a clinic. How do I say this?

Thanks,

Is it correct to say: I have been clinic.

Thanks.
