If there is such a insurance, it is news to me. Maybe I am not aware of its existence in this neck of the wood.
As far as my experience goes, a phone should come with limited product warranty. Retailers or service carrier usually offers extended warranty against loss and damages but this type of "insurance" typically is offered to you at a cost (few dollars a month) when you purchase a new phone during a service term, or sign up as a new customer at a service carrier. However, you don't have to renew it. As long as you don't cancel this warranty, the carrier will keep charging you monthly. I am not sure if your question relates to any of the above I mentioned.
