How to pronounce the ph in Stephen?

In Stephen, is the ph always pronounced as a v like in Stephen King or sometimes like an f? If so, is it a choice of the person or parents with that name or is there any geographical distribution?

I have never heard anything but "v". There is another English word like that. Posh Brits pronounce "nephew" with a "v" sound, I guess because the original French word had a "v".
