how to make letter asking for salary incrementt

am almost four years nearly to five years working in a real estate company. as i complete my three years i have receive 1000 of increament and now i nearly to complete two years i believe am entitle for increment as appreciation to my performance but i am shy to tell my boss about this. can you please help me how to make a light and considerable letter. can you please polish my letter.. appreciate your kind help.

Dear Sir,

I have been working to your good company for more than four years. which i believe i have proven myself to you my hard work, honest and dedication to my job.

I am confident that i can be consider as an asset to your company as i have updated all the tenants datas as always being a secretary even preforming receptionist with an ettique.

I would like to request you to please review my work performance for the past years. and request you to consider a salary increment as a reward and i am happy to accept another task you will be requiring to me as return.

I promise to continue to give you nothing but the best quality as your secretary & receptionist and bring more professionalism in your good company.
