How to make an appointment

Situation: "you want to go to the dehatntist for examination. you call him from home to make an appointment. What would you say?"
My sentence is: " I would like to make an appointment on Tuesday, the 21th, 2010 for an examnination, are you free on that day?"
Please tell me if my question is ok? or whether there is some other polite ways to express it. Thank you very much.
