How to gradually improve my english.

Hello people.I am posing a very dubious query to you (dubious from my point of view) because it is related to how to improve the use of english.I have started learning english for about 2 years, and since then i have been listening and reading a lot but it seems that i cannot get the full extent out of my skills.Emotion: sad i do always search new terms and look them up in the dictionary but sometimes i forget a lot of them and that is excruciatingly frustrating:( Could you please advocate me into undertaking something that is going to have long-standing effects? And there, exists also another aspect of my problem,of this plight i am constantly dealing with, and that is represented by the following: why cannot i speak fluently and every time i endeavour to do so i get lacuna and gaps between sentences:( I would really appreciate your help .Thank you.
