how to fill an application form.

Hi,

I am filling up an application form for employment and I have two questions.

One of the items says:
"Please give below the names of addresses of people to whom we may apply for information about your work, experience and attendance record and indicate your relationship. One of them must be your most recent or coronet employer."
And gives me three rooms to fill, one reads "Most recent employer" and the rest two reads "Relationship"

So my questions are:

1) For the Most recent employer, am I supposed to give them the name of the company that I worked for? or someone in the company I have worked with. I wonder this, because there's only one tiny space to fill a name.

2) What does this Relationship mean? Am I supposed to fill a name of my clients or something? or more like a friend of mine?

I haven't done this before and don't have the common knowledge that you people have. Any advice would be appreciated.

Yes, that was tough to explailn. Since the dead line was so close that I had to submit it with my best guess. (fingers crossed)
