How to explain a system error to user

Hi,

May I know where could get good examples of business emails/conversion for IT project manager and support?

For example:

1) Recently receive a complain from user that double shipments to oversea customer and they request return stock. It could be configuration error, even others send double orders to us.

'May I know' is not good English. Use 'Could you tell me'. There are no form letters for such specific instances, I think, but there are a lot of business letter formats HERE.
