How to build this sentence correctly?

Mr. A is employed as software engineer in B team within C department with Company D in a permanent and full time role from 1 January 2011 until 5 September 2012.

Is that sentence correct ?
  1. Should I use past tense or present tense here? Mr. A is or was employed ?
  2. Company D has many departments and each department has many teams. So is it correct to say "employed as software engineer in B team within C department with Company D" ?
Thank You.

I might chose different linking words, but this is generally correct. And, yes, I would use the past tense since you are talking about an employment that is now over. Here, for example, is one way to word this.
