How to ask permission for recall from annual leave

Dears,

Please help me to write to my manager, to get his approval for annual leave and for early return from vacation 2 weeks before.

Dear.....

I kindly ask you to give me annual leave for 30 days from 18/10/10 till 16/11/10, for my working period 23/05/09-23/05/2010, I would like to ask early return permission from vacation on 01/11/10, please approve.

We are not 'dears' and you should not use that term with your manager, either, if you wish to keep your job.
