How to ask people to provide more info

Hi guys,

I would like to ask how to properly write a note to ask our staff to fill in a form as detailed as possible and follow the format provided.

Currently, what I got was as below.

*Note

If it is possible and convenient, please answer the questions with the example format: YourAnswer (explanation)

But I am not sure if this is a OK expression.
