How should this be written?

He shook Mandy and Ellen's hands.

Is that written correctly? Or should it be "He shook Mandy's and Ellen's hands?"

I want to say that he shook both their hands.

The hands in question don't belong jointly to both Mandy and Ellen, so you need Mandy's and Ellen's hands . If Mandy and Ellen owned a single car though, you could ride in Mandy and Ellen's car. CJ
