How should the sentence be phrased?

On behalf of my family, I would like to convey my sincere appreciation for your good wishes.

I believe "on behalf of" does not include the speaker. If the speaker wanted to include himself, how should the sentence be phrased?

Technically, yes. But your sentence also says that you're conveying your own appreciation. The only reasonable interpretation is that both you and your relatives are appreciative. You could be direct with "My family and I sincerely appreciate your good wishes."
