A boy is sitting in the passenger's seat of a car, his father is driving. They're going on a fishing trip. When father wants to make a left turn, the steeringwheel jams and they get into a car crash. The father dies instantly; the boy is still alive, but his condition is critical. He's rushed to the hospital by ambulance. In the hospital it's decided that the boy needs surgery right away, but the surgeon says: I can't operate on him, that's my son!

I didn't know it was an old joke, but you're right, the surgeon could be the mother. There is another possibility however; maybe the boy had two fathers, then the surgeon can also be his other father
Of course Anne! Changing times affect all aspects of language, including jokes and riddles. I think when this 1st appeared it was at a time when it was naturally assumed that all surgeons were men, but this is a different interpretation. The boy could have a biological father and an adoptive / step-father, as you say.
