Does the modern "knock, knock, who's there" jokes go as far back as Shakespeare's time?
(Porter: knock, knock! Who's there, i' the name of Beelzebub? Here's a farmer, that hanged himself on the expectation of plenty: come in time; have napkins enow about you; here you'll sweat for't.
I'm not aware of a connection; and the use of 'knock, knock' in 'Macbeth' seems unlike the use in the joke. The scene is more akin to a masque, where representative characters are introduced one by one.
But children's games do sometimes have a surprisingly long history, so I wouldn't discount the possibility.
