How do you do? vs Pleased to meet you

00I've got a couple of questions about greetings when you first meet a person. I've learnt that the natural reply to "How do you do?" is "How do you do?". My questions are:-02br

001. It seems that I seldom hear people (including native speakers of English) say "How do you do?" on real business occasions in HK. Is this greeting phasing out in modern English actually?02br

002. If the person you first meet says "How do you do?", is it unnatural or wrong to reply "Pleased to meet you."?02br

003. Can I say "How do you do? Pleased to meet you" (i.e. both together)?02br

I realise that this was posted a decade ago but I would like to clarify that while you are right that 'How do you do?' is rapidly disappearing from common parlance, 'Nice to meet you' is never an appropriate greeting for someone and certainly not an acceptable response to 'How do you do?'. 'Nice to meet you' comes at the end of the first meeting because it indicates that you enjoyed the experi
