How do English people (living in ENGLAND!) address the following persons?

I am writing my final thesis now and I would like to make a questionnarie but I do not know whether my questions would be relevant to the people in England (they would be relevant in my country) that is why I am asking.
For example, if they (or you) do not know the shop assistant or the nurse but they are wearing a name tag with their full name on it, would they (you) call them by their first name or by titles like Ms/Miss/Mr?

Somebody has told me that in America everybody call people in different services by their first name but I am interested in the practices in England much more.

Hi, As a person from England now living in Canada, I usually don't address nurses or people who work in a store by any kind of name just because they are wearing a name-tag. ) Clive

Hi,

As a person from England now living in Canada, I usually don't address nurses or people who work in a store by any kind of name just because they are wearing a name-tag.
But I do, of course, speak to them politely (please, thank you, excuse me, etc.)
