How could that happen

Here is the conversation between me and an American friend (let's called her A). Could you please tell me if my answers are natural enough? if not, what is the better way? and obviously, the sentences said by my friend are different from what I wrote here. I just rewrote them based on the content I remember.

A: When I was in undergraduate, there is a professor who pushed the piano off the stage.

Me: How could that happen?

A: Something like the professor pushed too hard.

Me: I hope the piano was not damaged too much.
