How can i write a study leave.

Dear Madam,
Hope you are fine. I have been working in the octokhan from 2007.I can’t do anything when I joint in the office, You teach me every single item (office work; administration, sage, rule, office environment Etc.).I cannot explain properly but sill I am learning. It is my pleasure to work with you.
My monthly work of ....Com. is complete and IL Com. Annual account’s is nearly complete. My regular work is continuing process it’s never being finished. But my study time is almost finished; I have only 2 months remaining for the last chance. Here I explain that I have attend 5 (five) time in .. exam out of 6 (Six) time (Knowledge Level) but still I am incomplete 5 subject. I don’t know I can or not but I want to try my hart and sole.
In that case if you help me, I will would continue my work in daily biases or without pay leave during this period. My exam held on 8th December 2012 and finish on 15th December 2012.
