How can I write a letter requesting to change my study leave?

Question

I am working in a hospital and back in 2007 I decided to study my master degree. I was given a study leave until December, 2009 to go back to work. I did graduated in December, 2009 but, I did not report back to my employer because of many reasons, the most important reason is that I got excepted to study a PhD in same university and I was so busy in last 6 months working on my PhD proposal, also I am overseas and I totally forgot about my Deadline.
So, now I want to ask the hospital to extend my leave until December, 2010 because it's my nearest holyday.

By the way it was a leave without payment.

I know that I am asking too much but I couldn't do it myself.

Thank you so much for your interest in helping me, I do really appreciate it.
I was asked by the director of the HR to email the CEO to ask him for an extension.
So this is what I thought of:
Dear Dr. A,
I am writing to you regarding extending my study leave. I lift the city in 2007 in order to get my master degree and the deadline was December, 2009. I did graduate in that date with an average mark 73 out of 100.
(I am going to attach my certificates)
I have completed my Masters in vocational education and training. My Field of Study was focusing on vocational education and training for handicapped.
(I am working in a rehabilitation hospital).

I honestly forgot about the deadline and I have not got any notice from the HR. now for very important reasons I need to extend my study leave. First of all, during my last semester in my master I was accepted to the PhD program, so now I am working on my proposal which I am going to finish it around December 2010. Just want to let you know that my PhD in vocational rehabilitation. Dear Dr. A, as you see I am focusing my study in both my master and my PhD In the interest of the hospital.
(I have chosen to study this field; there were no obligations to the hospital).
I have been an employee since 1999 and I will be always loyal to my employer. I can't wait to come back to work to perform the best of my ability and I will be utilizing my knowledge to serve the hospital.
(By the way the CEO knows me and he knows that I was a hard worker).
Also my wife is studying her master in the main time and she also will finish her semester in December 2010.
I would be really grateful if you could approve my request to extend my study leave until December, 2010.

I would appreciate it and be grateful if you could grant me leave without pay till December, 2010, so that I can continue with my studies.
Also my wife is studying for her masters and to enable her to finish her studies it would be necessary for me to be her till she finishes as the embassy regulations.

any body can help me to write application for extension in leave which gonna finish in March,2012.
circumstances are: i took leave for my masters in 2010 ...i done my PGD ist in 2010 and progressed in in masters programme and rite now working on dissertation which will b final in May,2012....so i need to extension further till June.2012
