How can i write a good petition to drop a class?

I tried to drop on the dropping date but the system didn't process it through and apparently i'm registered in the class. I was working a NOC shift for full time and i also have family issues and i couldn't keep up with the Math class and its tremendous amount of homeworks. On top of that im diabetic and i get a lot of headaches, which makes it harder to concentrate on the class.
