Hello.

What are some possible replies to "How are you doing?"
Is it wrong to reply with answers like "I'm fine, how about you"?

If it just seems like speaker #1 is saying 'Hello', then that reply is fine. If it's a context where you think speaker A is really interested in your personal situation, then you could consider a more detailed reply. eg Not so good.
