1) I booked a room for the 15th to the 17th.
2) I booked a room from the 15th to the 17th.
3) I have a reservation for the 15th to the 17th.
4) I have a reservation from the 15th to the 17th.
5) I have a reservation for checking in on the 15th and checking out on the 17th.
Sentence #5 is unnecessarily long. Use one of the others, but specify the month. If you're talking about the current month, use "this month".
