Holiday request

This is a holiday request which my friend asked me to translate into english, could you please check and correct it?

"I would like to kindly request (for?) 11 days of holiday.

The reason of my request is that I need to go to Poland from October 20th to November 4th.

All Saints Day is on the first of November in Poland. Both of my grandmothers died this year and I was not able to take part in the funerals - hence the time of my visit.

I also wish to have a dental check-up during my stay.

In my(or the?) free time I am going to prepare the Restoration of the firm xxx area project as I am an landscape architect.

I would be grateful if my request is considered positively."

It is wrong. If anyone is kindly, it is the other person, not the one who requests; however, 'kindly' is not used in AmE/BrE English business letters as it is in Indian/Asian English.
