How is he doing? - normally used if somebody has been depressed or ill for a while.

He is under stress in some situation (wife is ill, child is missing, etc.). "How is he holding up" asks if he is doing o.k. given the circumstances: how is he doing, how are his nerves, is he handling the situation ok, etc.
