To my ear, using this adjective in briefly describing a cafe seems rather pretentious.

Hmm, I don't see it that way. In the UK, cafes are often quite modest places, so usually they wouldn't be very refined, but if one was then I don't personally see anything pretentious about calling it so.

Perhaps I feel this way because it sounds to me as if the speaker is setting himself up as an arbiter of taste.

In my mind's eye, I see the Monty Python cast mocking someone who presents himself as an authority on "reefaynement".
