hi, do you think my letter is formal or not?

Dear Mark

It was nice to meet you briefly this afternoon. I would be grateful if you send me my orientation timetable. In addition, I need information on module selection guidance which is "Guide to the Biosciences Undergraduate" . And also could you please send me the application form that I have to fill it for my optional modules.
