Hi all Appreciate if you could help me to correct my grammar in this letter.Tq

Ref. :- Your show cause notice dated 25/07/13 regarding ‘c’ form information
Respected Sir,
We refer to your notice relating ‘C’ Form sr. no. 056 Dated 11/07/13 in respect of foreigner Mr. ONO TAKASHI,
Mr. ONO TAKASHI was arrived in hotel dated 11/07/13 at 20.33pm for 5 days but for sum company work hi extend his stay for more 5 days. But apparently we did not inform to your office
I would like to apologize for the mistake that we have made. & in future we will be more alert regarding same matter.
We hope no disciplinary action will be taken against us as we already did the assessment. We assure you that this will not happen again.
