I really need some help with a doctor's handwritten description of a problem, but I can only recognize a few words of them. I'm wondering whether anyone can help me with it and let me know what has been written in the document (Sections 1, 2 and 3)?
I can see the second link, but doctors' handwriting is famously illegible. If you want an actually accurate interpretation of the document you're going to need to go to a doctor, nurse, or pharmacist. I'll do my best, though.

One question I still have is do you have any idea what the words in the left part of Section 3 means? It's above "for intra-articular injection"? It's "Knee pl(??)/??". I really have no idea at all.
