Which of these sentences are correct?

1. He is the reason that made me decide to come back and play soccer.
2. He is the reason I (have) decided to come back and play soccer. If possible, is (have) also possible?
3. He is the reason for my decision to come back and play soccer.

Thanks. So, it may be more recent, but importantly it is a past action that has a present consequence. In other words, the present statues of a past event, which all depends on the context. Am I right?
