Help to write a letter for issue student visa for London

Coul´d you please try to help me to write a 'Formal Letter' ?

It will be a letter from a Guest of the Bank where I have my money (found extract ?)

Like this:

"The purpose of this letter is to inform the bank account of GUILHERME CAMPOS for issuing the student visa on London Embassy.

His account numbers are as follows:
Agency: 2274-8

Poupança (?)

The funds of account have the amount below:

R$ 00.000,00
Should you have any questions, I can be reached at my above address or by e-mail at Emotion: email and adress

Hi,
It's not clear to me what is happening here.

Does G. Campos want a visa?
Does the bank account belong to G. Hermos?

Are you a bank employee?
When you say 'guest', do you mean
customer/client'? 
Are you a customer/client of the bank?
