Help on tenses

Hi. Please help.

1. When you have two past perfect tenses (as it seems for the sentence below) in a sentence, whether as part of an adjective clause (as it seems here) or in a main clause, is there any chance one could be a past perfect and the other a past tense?

They spotted a thirteen-year-old who had fallen and hit his head.

2. Would you say present perfect tenses are correct for the underlined parts? For the former sentence, I feel two present perfect tenses in a row aren't attractive (?) stylistically, if not wrong. What do you think?

He has written many books in the past, notably ones dealing with child development. Recently he wrote (has written?) a book named ...

In my session, I have been praising my friendly neighbor for his recent help. And someone suggested (has suggested?) to write a thank-you letter to him. I wrote (have written?) it and here it is. Could you correct it?
