Help On My Sick Leave Request

I would like to write a leave request for visiting my father in the hospital overseas. I actually want to ask for 4 weeks off to spend more time with my father, but am afraid that it won't be granted. The air ticket is very expensive for a short stay. Please give me some suggestions. Here is my draft:

I am sure you still remember the time that my father got a massive stroke and I had to send him back to China seeking for a long-term care. It has been two years since the last time I saw him in the hospital in Beijing. Thanks to the doctors and nurses! He is still with us and his condition did not get worse compares to when he was just admitted into the hospital. I would like to request to take three weeks sick leave from January 24^th, 2011 to February 11^th, 2011 to visit him in the hospital in Beijing.



I am not 100% sure yet about the exact dates in my request because it also depends on the availability of the air ticket. I will inform you if there is any change. Also, I am willing to put in extra efforts to reimburse the delay at work once I come back from my trip. I really appreciate all of your help.
